The registered nurse plans, organizes and directs home care services and
is experienced in nursing, with emphasis on community health
education/experience. The professional nurse builds from the resources
of the community to plan and direct services to meet the needs of
individuals and families within their homes and communities.
Completes an initial assessment of patient and family to determine home
care needs. Provides a complete physical assessment and history of
current and previous illness(es).
RN (CASE MANAGEMENT)
The Case Manager/ Rehabilitation Consultant is responsible for providing
management services to assist individuals in reaching pre-injury level
functioning and/ or maximum medical improvement.
Case management activities include but are not limited to coordination of medical care, counseling and placement.
Graduate of an accredited school of nursing. New graduates and
Registered Nurses (RNs) with experience within the last 2 years will be
accepted and must pass competency evaluation.
